Card Authorization Form

  1. Download and print this page. A form is required for every card used.
  2. Fill out the form, place you drivers license and card in the blocks. Must by physical card with number and name on the same side.
  3. Take a photo of the completed form and text to 254 681 2121.
  4. The password for the Card Processing page will be sent to you upon receipt of the Card Authorization Form.
